We received almost 800,000 euros in funding from ZonMW through their Rational Pharmacotherapy program. With this grant, we aim to optimize individual propofol dosing for anesthesia induction. This project is an exiting international collaboration between the anesthesiology departments of the UMC Utrecht (NL) (with Judith van Waes, Teus Kappen, Richard Bartels, Lisette Vernooij), the UMC Groningen (NL) (with Michel Struys, Douglas Eleveld, Jeroen Koomen, Jop van den Berg) and the University Health Network Toronto (CA) (with Justyna Bartoszko, Tariq Esmail, Stuart McCluskey).Propofol is the most commonly used drug to put patients to sleep before surgery. Despite extensive use since almost 30 years, the correct individual induction dose is unknown. Although there is a pharmacological model to calculate the dose for 'sufficient sleep', this model does not take into account all known factors. Side effects on the cardiovascular system (too high a dose) are predicted with other models. Therefore, most anesthesiologists use their experience to calculate the induction dose, and low blood pressure at the start of anesthesia is common (25-50%). Low blood pressure is related to kidney and heart injury after surgery.In this project, different pharmacological models for the initial dose of propofol will be combined and optimized using existing data from electronic patient records. After improvement and testing, the result will become available as an easy-to-use dosage advice algorithm that can be implemented in the electronic patient records system.ZonMw, University Health Network, UMC Utrecht, UMCG

The Green Team at Sophia Children's Hospital is expanding their paracetamol challenge 💊After a successful start last week in the Children's ICU, the challenge will be extended to the Neonatology ICU. This will make a greater contribution to sustainable care and further cost savings.Children's hospitals can lead the way on providing sustainable care. Initiatives like the paracetamol challenge are examples of ways that clinicians take take the initiative, starting where they have agency. Every action counts! About the project ➡ Intravenous paracetamol has a major impact on the environment and is much more expensive. In addition, intravenous administration is not always necessary or better. The challenge is therefore to administer 25% less intravenous paracetamol. In the Children's ICU, for example, 70% of paracetamol is still administered intravenously. That must and can be done less.Of course, the patient's individual situation is taken into account. In case of acute or severe pain, we start with intravenous paracetamol. If the patient is well adjusted, oral or rectal paracetamol can be used. With this challenge, we want to raise awareness for timely and considered switching.

🎗 Adressing obesity to reduce cardiac arrythmia!Last week the first patients were randomized in a new clinical trial: DUTCH-WAIST. It is a great example of a collaboration between many DCVA partners. Data from the DutchAF registry, subsidized by ZonMw and a collaborative effort of Nederlandse Vereniging voor Cardiologie (NVVC) Hartstichting Nederlandse Hart Registratie (NHR) Federatie van Nederlandse Trombosediensten and Dutch General Practitioners Network,identified obesity as a major driver of atrial fibrillation. DUTCH-WAIST, conducted in 18 hospitals, aims to study whether atrial fibrillation decreases when patients lose weight and adopt a healthier lifestyle. A game changer in addressing disease burden through weight management, and a great example of a registry-based trial!The study was a winner of the 2021 WCN-DCVA Pitch, with the majority of the DCVA partners present as a jury. WCN offered the winner a start-up subsidy from its Withering Stichting Nederland (WSN foundation) and provides project and site management. The Hartstichting has awarded a subsidy from the clinical trial program to Rijnstate as a sponsor. 🖥 🏥 Good registration and (re)use of data for research are vital to reduce the cardiovascular disease burden. The DCVA therefore supports the Nederlandse Hart Registratie (NHR) and partners in their initiatives to promote and improve research possibilities and data-linkages for cardiac care. One of these projects, mentioned in the post below, is the Heart4Data consortium.The Heart4Data consortium aims to develop a sustainable infrastructure for cardiovascular registry-based research in the Netherlands 📊 . Heart4Data is funded by the Hartstichting and ZonMw and is a collaboration of many partners of the DCVA. Neurologists and psychiatrists must start to take a clear stand on long covid, postvac, myalgic encephalomyelitis and functional disorders. To be sure, LC, PV and ME are organic diseases and no functional neurological disorders, never have been and never will be.I think many functional neurological disorders are in reality organic. FNDs are merely a convenient bucket where some neurologists, psychiatrists (and other somatic colleagues) dispose of all the more difficult patients claiming to be physically sick. This misdiagnosis and mistreatment to explain ME symptoms such as cognitive dysfunction, fatigue, exertion intolerance, tachycardia, dizziness, pain and muscle weakness is the medical gaslighting patient advocates like me are fighting. Over the past four years, not one somatic physician has bothered to biopsy my muscles, which are quite obviously breaking down and wasting. Nobody cared to find out why my lung function is 68% although I have before Covid been able to run 10 marathons (and in the first test still had a 100% function). Nobody tried to explain why my deep and REM sleep no longer function, why my immune system is dysregulated, why I have tachycardia, why my body can no longer break down histamine or transport oxygen to my muscles.Infections such as EBV or SARS (and many more), certain treatments (like chemotherapy or antimicrobials), and vaccines (Covid, flu, shingles, etc) can trigger the immune system, which dysregulates all organ systems and functions. I will not rest, not calm down, not accept psychosomatic diagnoses and cognitive behavioural therapy and antidepressants as treatments for our primary symptoms, we will not accept harmful graded exercise as treatment. I will continue to behave like a hyperactive hysteric woman, will insist that I have an organic disorder, that I need an adequate diagnosis and adequate treatment.

𝐈𝐭 𝐭𝐚𝐤𝐞𝐬 𝐭𝐡𝐫𝐞𝐞 𝐭𝐨 𝐭𝐚𝐧𝐠𝐨: 𝐟𝐚𝐭 𝐯𝐞𝐬𝐬𝐞𝐥 𝐜𝐫𝐨𝐬𝐬𝐭𝐚𝐥𝐤 𝐢𝐧 𝐦𝐮𝐬𝐜𝐥𝐞 𝐚𝐭 𝐭𝐡𝐞 𝐨𝐫𝐢𝐠𝐢𝐧 𝐨𝐟 𝐝𝐢𝐚𝐛𝐞𝐭𝐞𝐬With this research we aim to elucidate the role of fat around microvessels in muscle in type 2 diabetes and muscle weakness. We hope to discover that this fat controls contraction and glucose uptake of muscles, and its functions can be normalized in type 2 diabetes by locally treating it with medicine. To achieve this, we will make images of this fat and sugar uptake in muscles of healthy people and people with early diabetes, and precisely characterize gene expression in muscle fat. We will study the effect of muscle fat on sugar uptake and strength in human “muscles in a dish” built from muscle cells, human microvessels and human muscle fat. Finally, we will remove muscle fat in obese mice, or treat it with a smart coating containing an anti-inflammatory drug.
